Understanding the HOPE Mandate—and What’s at Stake 

The HOPE initiative from CMS (Hospice Outcomes and Patient Evaluation) represents a major step toward greater transparency, quality measurement, and accountability in hospice care. And while it's not mandatory yet, its rollout is already reshaping how leading agencies prepare for regulatory success. 

So what exactly is HOPE—and why does it matter now? 

What HOPE Changes: 

  • More structured, in-stay assessments: Providers must capture patient condition and service delivery throughout the stay—not just after. 
  • More emphasis on interdisciplinary collaboration: Teams need to communicate more often, and across more disciplines. 
  • Greater visibility into quality: HOPE aims to create a more data-driven picture of what great hospice care looks like. 

 

What That Means for You: 

To succeed under HOPE, agencies will need to rethink how their teams work—not just how they document. 

  • Workflows must be fast and traceable. There’s no room for lag time when real-time patient needs must be documented on the spot. 
  • Task tracking needs to be visible. Supervisors must know what’s been completed, what’s outstanding, and what’s at risk. 
  • Collaboration needs to be consistent. Interdisciplinary coordination will now be measured and must be demonstrated in your processes.
